All-on-4, All-on-6 and full-arch implants

A complete set of fixed teeth on a small number of implants. For people with no teeth left in a jaw – or teeth that can no longer be saved – it is one of the most life-changing implant treatments.

What the names mean

“All-on-4” describes a concept in which a full arch of teeth is fixed on four implants; the back implants are often placed at an angle to make the most of the available bone. “All-on-6” uses six implants. Some dentists use more, depending on the jaw. The names describe concepts – what matters is the plan made for your mouth.

Four or six implants?

More implants spread the load across more points and give a reserve if one implant ever has a problem. Fewer implants can mean less surgery and sometimes avoid bone grafting. Upper and lower jaws also differ: the upper jaw usually has softer bone and the sinuses close by. A dentist weighs these factors after looking at a CBCT scan.

Temporary teeth, then final teeth

  1. Diagnosis and planningImaging, discussion of fixed versus removable options, extraction plan if teeth remain.
  2. Surgery dayRemaining teeth are removed if necessary, implants are placed and – when stability allows – a fixed temporary bridge is attached within a short time.
  3. Healing phaseFor several months you wear the temporary teeth and eat a softer diet while the implants integrate.
  4. Final bridgeA stronger final bridge is made, often on a metal or zirconia framework.

That makes at least two trips for most travelling patients. Plan your time realistically – our guide one trip or two? explains why.

Questions worth asking

Common questions

Is All-on-4 suitable for everyone without teeth?

No. It depends on bone volume, health, bite and expectations. Some people are better served by more implants or an implant-supported denture.

Do I leave the surgery with teeth?

Often a fixed temporary bridge is attached soon after surgery, but only if the implants are stable enough. The dentist decides this on the day.

Can the bridge be taken out?

A fixed full-arch bridge is screwed in and can only be removed by the dentist, for example for professional cleaning or repair.
